One of the most important hormones in supporting a healthy pregnancy is Progesterone. This hormone is produced by the ovaries in the first trimester followed by the placenta from the second trimester through birth. Immediately after a woman ovulates her ovaries produce more Progesterone to support implantation into the uterus. If there is a deficiency of Progesterone, implantation will not occur successfully resulting in a miscarriage.
In certain patient populations, we provide Progesterone supplementation in the first trimester to help support the growing fetus. We also screen women for cervical shortening during their routine anatomy ultrasound between 18-20 weeks gestation. We have found that a short cervix (less than 2cm in length) is a significant risk factor for preterm birth, which is the leading cause of morbidity in newborns. If cervical shortening is found, one of the treatments is Progesterone supplementation. Progesterone supplementation in patients with short cervix has been shown to prolong pregnancies allowing babies to mature longer in utero. The closer to term a baby is born, the lower the morbidity rate. I have used Progesterone supplementation on a number of my patients with great success.
Abortive medications such as Mifepristone, aka RU 486, work by blocking Progesterone reception causing the growing fetus to “detach” from the uterus resulting in a medical abortion. Often women feel regret after taking abortive medication and seek out abortion reversal support. When trying to “reverse” these medications we use higher doses of Progesterone in an attempt to saturate the Progesterone receptors and support the pregnancy through the first trimester.
